I'm afraid the question is slightly confusing as worded, however, from what I understand you suffered a stroke 10 years ago and have been prescribed Ecosporin to take daily.

This is likely because the type of stroke you suffered was considered what we refer to as a SMALL VESSEL ISCHEMIC stroke.

It was not caused by a BLOOD CLOT or anything of that nature and therefore, ECOSPORIN is quite a satisfactory drug to take in that situation.

No testing is necessary as it is not really an anticoagulant and in fact, more modern blood thinners of that type don't always need to be monitored anyways.

Therefore, it doesn't surprise me that your doctor is not doing any tests if things have been stable and you've suffered no further stroke symptoms or strokes themselves.

You should continue to take the medication. As far as the question regarding the "blood clot under the tongue and it ruptures".

I don't believe that you are truly experiencing that sort of complication since the arterial supply of the tongue is not in a place where rupture into the mouth does not really happy.

There is a large vein that supplies drainage of blood from the tongue but blood clots don't really form and then, burst in that area either.

However, if you've developed an ULCERATION of some sort that may be from something entirely different and you should show that to your doctor for an opinion and treatment. If you would like to take a digital photograph of this lesion and upload to my attention I'd be happy to look at it and give you an opinion as to what it may be.

It is highly unlikely that such ulceration under the tongue as you describe is due to the stroke you suffered.

The issue you describe doesn't seem to have any relation to stroke whatsoever, it doesn't cause mucosal ulcers such as you to describe. It's hard to understand what the issue is without looking at it, a consult with a dentist or an ENT specialist would be the best step.
