How To Treat Breathing Problems Accompanied With Weak Heart?

I had a dib. installed 2010,I am still very short of breath. the brething problem started in 2009. My hearth doc. said that my heart was in very poor shape. I had a triple by pass in 1997. Felt good until 2009. Today I feel good , but I am just to short winded.
