Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

The gestational sac( sign of uterine pregnancy) may not visible in fifth week. It become more prominent in seven weeks. The other possibility is extra uterine pregnancy which is nothing but pregnancy somewhere othe than uterine cavity such as tubal pregnancy. Do a follow up scan after two weeks to reassess the sac. HCG is non specific and it is secreted by other tissues. Beta HCG is pregnancy specific and is sure sign of pregnancy.

Hope I have answered your query. Let me know if I can assist you further.
