General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

At 49 there is a wide range of causes; from benign causes such perimenopausal bleeding, uterine fibroids, endometrial hyperplasia to rare and severe causes such as endometrial or cervical cancer. You need gynecological consult. Provera is empiric treatment, will treat the symptom but not the cause in most cases.

Hope I have answered your query. Let me know if I can assist you further.
