Pediatrician, Pulmonology 's  Response
HI..Greetings from Chennai...there are few possibilities -
1. Ear infection - otitis media
2. Evening colic
3. Acute CNS infection/ Pyogenic meningitis

The 1st two are not dangerous, but the last one is. Fever of a 3 1/2 month old kid with inconsolable cry, the third possibility should definitely be considered and the baby might need an expert Paediatric opinion and investigations including a CSF analysis.
