Dear Sir/Mam,
i totally understand your concerns about your daughter and the pregnancy. You are dealing with two lives instead of one.
Since your daughter is pregnant that makes her case different from a non pregnant lady.
The problem is not the fever, the problem is what infection does your daughter have that is causing this fever.
The following cases should be taken into consideration:
1- If your daughter has
flank pain, then
pyelonephritis is the most likely diagnosis. urine analysis and urine culture must be taken anyways. If there is a UTI by clinical suspicion your daughter must be admitted to the hospital to receive intraveinous antibiotics until she becomes afebrile then continue
oral antibiotics for a total of 2 weeks. and then she must be put on a suppressive therapy for any possible UTI during this pregnancy.
2- if your daughter has flu like illness it is recommended to make sure she doesnt have an
influenza A or B, if that is the case a treatment with
tamiflu is needed. Otherwise watchful monitoring is enough.
3- if your daughter has cough, a chest X Ray must be done with abdominal shielding to protect the baby from the XRays. if there is a suspicion of
pneumonia then she must be admitted to the hospital and receive intraveinous antibiotics until she becomes afebrile.
4- any other infection is possible during pregnancy causing fever. according to the etiology that is causing the fever the management is different. However the fever itself is not as dangerous as the infection causing the fever. So the source of infection must be identified and treated. I mentioned the most common types of
infection during pregnancy however a detailed history and physical exam can lead us to this source.
This should be taken seriously.
Best regards and get well soon
