How Can Fatigue Be Treated?

My brother had a procedure to treat an arterio-venous fistula in the brain, had complications and full brain surgery. He suffered severe reperfusion injury and now 5 months out, after physical, speech and occupational therapy still suffers severe fatigue. What is the best treatment for the fatigue? Is physical and mental exertion to the point of fatigue detrimental, or can it be rehabilitative. He is back to work, but needs to sleep up to 14 hrs a night to feel rejuvenated.
