Hello Doctor Koregol I have a query. My uncle 55yr old is suffering from deteriorating heart and kidney functions. Patient has a history of high diabetes, and asthma along with this. Was brought in to hospital on life support system 3 days back as he had slipped into coma. He was in the ICU. Wasn't able to urinate and after an initial dialysis the urea and creatinine levels lowered and he regained conciousness. However his cardio is not too good with heart functioning at 15% only. His heart muscles are think and that is resulting in low pumping of the blood. Thereafter moved to ward for 2 days and soon after complaint of non urination and was in a disoriented state of mind and was moved back to ICU with a second dialysis happening yesterday and with that the Urea and creatinine coming down and he regaining normal composure. His BP is 101/66 and stable and so is urea, but however under medicines. Has been prescribed lasix, digoxin & fragmin at present and he continues to be in ICU. The nephralogist is stating that the kidney is functioning properly but due to the weak heart muscles not able to pump blood. The cardiologist states that a surgery for placing a pacemaker or likewise is ruled out because of kidney issues. Can you please kindly suggest a course of action in this situation. The kidney problem is due to poor perfusion from the failing heart. The major and underlying problem is the heart - advanced heart failure. With a back ground of Diabetes and asthma / and an ejection fraction of 15% - sorry to ssy - the prognosis is extremely poor. What is the cause of heart disease? In such situations, only a heart transplant may be the alternative and he doesn't seem to be a fit candidate... Discuss with the treating doctor
