hi,
as you have mentioned your symptoms of joint pain and previous history of raised ESR it indicate some form rheumatological/ connective tissue disorders.

In these diseases there is involvement of one or multiple joints (as in your case) also there may be skin involvement (various kind of rashes may develop) in some severe forms there may be involvement of internal organs like kidneys, heart, lungs there might be bleeding tendoncies.

but don't become panic, detailed evaluation required for diagnosis.
Basic tests like Complete blood counts, blood tests related to liver, kidney, xray of chest, urine test, ESR are help full.

More specific test like RA factor & anti CCP (citrullinated cyclic peptides) indicate rheumatoid arthritis, ANA (antinuclear antibodies) and if it comes positive then ANA blot assay, complement levels C3, C4 need to be done.

Depending on these tests and patients symptoms various diagnosis can be made. ( rheumatoid arthritis, systemic lupus erythmatosus, systemic sclerosis, sjogren syndrome)

treatment should be directed towards underling causes and complications.

usually these diseases occurs in female more than male.

Though other causes of raised ESR like long duration of infections like tuberculosis could be possible.
