Can A Sphincterotomy Help When Botox Injection In The Sphincter Muscle Didn't Improve Recovery From An Internal Anal Fissure?

I am 78 years old and have had an internal anal fissure for about a year. I have recently had botox injection in the sphincter muscle. It had no effect. I have been advised that a sphincterotomy or an anal advancement flap are my two next options. I am leaning toward the sphincterotomy. Do you have a recommendation? My email address is YYYY@YYYY . -Get treatment for chronic constipation or ongoing diarrhea.
-Sitz baths, or hip baths, can promote healing of an anal fissure.
-By soaking the rectal area in a tub of warm water two or three times a day for 10 to 15 minutes you can clean the anus, improve blood flow, and relax the anal sphincter.
