Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

I have to ask how old is the child?
In my practice, I don’t use mefenamic acid as it may cause pain or ulcers in the stomach. If you use, it is better after meal.
Have you tried ibuprofen for high fevers?

As for your question, I don't see a reason of not combining ibuprofen with paracetamol as they belong to a different class. Anyway, I would suggest ibuprofen and paracetamol to control the fever.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Mersila Qahajaj
