Your problems can be caused by fungal infection of this area or by prostate inflammation. This area is constantly moisturized what makes great environment for fungi to grow. Such infection can last for a very long time and can be hard to treat. It is more possible if you have any redness, skin patches or bumps in this area. The other possibility is prostate inflammation. Ultrasound or blood tests can be normal. However PSA level should be elevated. Sometimes it is hard to diagnose it so you may try Ciprofloxacine 500 mg twice a day treatment to see if it help.
In case of fungal infection, dermatologist consult is advisable.
Hope this will help. Regards.
