Are Fibroids In The Right Ovary Suggestive Of Cancer?

I am 46 yrs old. I have been having irregular heavy periods. My most recent period was just November 23 and before that was November 2. In the last month, I ve gained 12 pounds without changing eating habits and I exercise regularly. This weight is in my abdomen. I m bloated and look 4 months pregnant. I m having pain and tenderness in my lower right pelvis. I went to see my CNP and she ordered an ultrasound. We did a pregnancy test to rule that out but it was positive. She repeated it and still positive. I went for the ultrasound and there is no sac. They said they see two fibroids on my right ovary. Could this be cancer? I have to go in for blood work tomorrow. I want to be my own advocate but not sure if this is normal with fibroids or if I should push tests for cancer.

Hello,

Given your symptoms and irregular bleeding there are two pathologies that I would consider. One, it can be due to fibroids as they are one of the most common disorder. Fibroids in the endometrium are prone to cause heavy bleeding. If it's a fibroid with symptoms then surgical management is best. But again you won't be fertile after surgery. You can also opt for medical management. It's subjective and your physician cab decide what's best for you depending on your past history.

Second, As you tell me that the pregnancy test was positive twice I suspect choriocarcinoma as well. This is very subjective because I'm not sure if your doctor tried the urine pregnancy test or blood measurement of HCG. if it was urine then you can very well eliminate my second hypothesis. If it was a blood HCG measurement and if the HCG levels are way above normal limits without a sac in ultrasound then it's advisable to do a biopsy and rule out this.

If you are sexually active and had recent sexual intercourse where you believe that you might have chances to be pregnant then missed abortion or an ectopic should also be ruled out. I'm giving this last considering your peri-menopausal age.
