General & Family Physician 's  Response
Hi Rajagopal, Welcome to HealthcareMagic Forum.
The urine routine test result of your Mother suggests Urinary Tract Infection, take her to a Urologist as she may need antibiotics. Since she has loose stools, better she gets a Urine and Stool culture done. She needs to exercise and take her Blood Sugar and Hypertension medications regularly.

Good Luck.
