Brief Answer:
intestine infection, needs GI antibiotics and supportive care.

Detailed Answer:
Hello and welcome to Ask A Doctor services.
I have read your query and here is my advise.

* As per my clinical experience, the abdomen spasm since 20 days on & off
is seemingly related to intestine infection through some sort of bacteria or
virus most likely.
The blood in flesh pan can be related to peri anal skin friction from hard
stool ( more likely possible ) or related to intestine infection ( less likely as
it is not mixes with stool ).

I would recommend following measures for her best recovery at earliest time period

- Allow her more liquids to be taken from mouth as boiled water, lemon
juice, coconut water, fresh butter milk etc.
- Prefer to provide easily digestible food stuffs as rice, dal, organic fresh
fruits, cereals, pulses, legumes.
- Avoid dairy fat, pasta, spices, aerated drinks, ice cream, non veg if
consuming.
- Get prescription of GI specific antibiotics as metronidazole, anti spasmodic
medications containing dicyclomine , probiotics as lacto bacillus to support
the gut flora and help kill bacteria causing underlying intestine infection.
- Check her hand hygiene before and after eating as well as wash room visit.
- Do not allow her to put toys inside mouth ( if doing so )
- Apply xylocaine jelly ( 2 % ) over peri anal area to smoothen the skin and
avoid friction bleed.

In my opinion, above measures will surely reveal best outcomes.
Do feel free discuss further regarding her issue.

Wish her best recovery and health.
Regards.
