Brief Answer:
Less absorption of iron in intestines due to reduced acid levels.

Detailed Answer:
Hello and thanks for choosing "Ask a Doctor" service for your query,
Have seen your details and reports attached by you,
Firstly you are having microcytic hypochromic anemia (size of red blood cell is small and hemoglobin contents are low).
I would suggest you to take iron polypeptide tablets 12 mg once daily for 1 month followed by 6 mg once daily for 2 months. Minimum 3 months time is required to replete your iron stores.
Take this tablet empty stomach in morning.

Secondly your Total cholestrol / HDl ratio is high which keeps you at risk of future cardiac disease. The only way to increase HDL is to do high intensity exercise 30 minutes 5 days a week.

Vitamin D is slightly low. Take Vitamin D3 60k capsules once a week for 4 weeks and once a month for 4 months.

Rest every thing is fine. Mild variation of numbers or few tests have no clinical significance unless patient is symptomatic. To be XXXXXXX doctor treats symptoms and not numbers.
Apart from bleeding a number of things could cause Iron deficiency anemia like worms in intestines,Diet not rich in iron. However in you case it could be a result of use of PPI and fundoplication. Iron in food is present in a form which could not be absorbed. The hydrogen ion present in stomach acid converts it into absorbable iron form. As a result of PPI and fundoplication acid formation is suppressed which directly reduces iron absorption.

I hope I was easy to understand.
Please feel free to ask follow ups.
Thank you.
