Question: hi,

im 31, male, non smoker, i dont have diebeties as far as i know but I believe elevated glucose levels. my a1c i belive was 5.6 or 5.7 guessing since its been a while. I also have an irbbb and pacs relatively often. ive had annecho and stress test probably 3 or 4 years ago now that came back woth no concerns. its been a while since being to the Dr as well. Tonight at around 9 i had a coffee martini, small califlower XXXXXXX pizza and a handful of vegetarian appitizers.

Other than that I just had a ginger caffeen free tea before bed.

Last night i had a dream of my heart beating all over the place and slowly stopping and thinking I was going to die plus chest pain. I ended up dreaming it over and woke up from that nightmare reaizing i was sleeing on a pillow which was hurting my chest.

i certainly was somewhat anxious hoping to not go theough thebsame tonight so I went to bed and laid down. I noticed my pulse was somewhat rapid id guess 80-90s and the actually pulse i felt was short quick bursts. While i had been feeling it at my left or right (not both at the same time) corotid neck area i found my heart rate slowing and seemingly feeling softer and slower, it got deom what i recall very soft/faint, i felt a slight tightness in my chest and quickly turned from my left side to on my elbows and started to panic which certainly brought my pulse back up. im not aure how much i was actually breathing during this as i was laser focused on my heartbeat.

i do frequently find depending on positions that my vegal nerve gets stimulated easily causing breaks in my heart beat and pounding sometimes just as an fyi.

i was certainly a litte bloated and had gas that hurt around my stomach area off and on surely from the food I ate.

i took two drs best chealated magnesium maybe an hour ago now.

With all of that said and for background info, im scared that I essentially nearly died or suffer from some event that could have damaged my heart? Or does the combination of all of those things produce that kind of effect. i feel fine since and it was all only in that moment. with my deams being awful just from being aware of my heart before bed, ive been concerned something has been happing during those dreams and i was simply feeling it in the dream while it was happening.

I apologize ahead of time for probably aounding like a crazy person, it is just terrifying and its only happened the past couple days. ive had a lack of sleep which hasnt helped and increased palpitations. I just dont know how my pulse could feel scary soft and slow down.

thank you for your help,

Matt

happy new year

Brief Answer:
Anxiety is very likely the cause of the symptoms you have- details below

Detailed Answer:
Hello,

I carefully read your query and think that your symptoms are mostly related to anxiety. Patients when having anxiety may feel the heart be fast and have chest pain and also may have difficulty breathing and in some cases can also feel shaky and numbness and if this lasts can also faint. It seems to the patient as if they are going to die and if their heart is having something serious. In fact it is the anxiety hormones that stimulate the heart and cause the symptoms and the heart generally is not affected by this anxiety episodes.

I would say you don't need to worry of your heart, the heart problems happen mostly with exercises and not in rest. If your symptoms are at rest and last only few minutes to half an hour and then you feel fine, it is anxiety. Not sleeping well does affect your emotions and may make anxiety worse so make sure you have good sleeping hygiene (go to bed in the same time and leave it in the same time each day, stay in bed only when you sleep, try to avoid caffeine containing drinks 4-5 hours before bed time like coca, coffee, energy drinks, etc, do some exercises during the day but not before bed, do some walking in quiet green areas, etc).
Drugs can also cause problems with anxiety.

If you notice you are not improving in 2 weeks or you have any worsening then very likely you will need to see your doctor and have treatment for anxiety to help you with the symptoms. Maybe your doctor at that time will also do some tests like CBC and TSH, FT4, etc.

I hope I answered your query. Let me know if I can assist you further.

Happy new years as well.

Kind regards,
