Hi Doctor, For The Past 9 Months I Have Been

Question: hi doctor,

for the past 9 months I have been having weird erection feelings. it started one day where my penis felt weird while it was flaccid. could not pump it internally (if that makes sense) and i tried masturbating and couldnt get an erection. I basically ejaculated within seconds with a flaccid penis. the next day i went to a urologist and he prescribed some type of ED drug i think it was cialsis or viagra and i was able to get a proper erection. ever since then i was able to get erections naturally but sometimes they felt weird and sometimes if i wasnt manually stimulating it it would go dwon quicker than usual. sometimes it stays there until ejaculate. i also wake up to fewer morning erections. i used to wake up to them every night but now only 1-4 times a week and sometimes the morning wood is weak and now it goes away fast whereas before i could go to the bathroom and pee and still have an erection. what is the cause of all of this? ive seen a few doctors and wrote to so many doctors online and they are all saying its anxiety but honestly i'm so mad because they are all saying it is psychological but i can feel a difference in my erections and penis in general......
i' so fed up with doctors that they are not taking my problem seriously. It makes me wonder if they completed skipped sexual dysfunction lessons in med school or if they were taught that if anybody under the age of 50 comes to your office and says they have ED just turn them away and say they have anxiety...

i just want my life back. yes, sometimes i get good erections but other times i dont. it's not adding up. no doctor has even did a physical exam on my erection...they just did a pathetic blood test to check for my hormones but i think it may be a physical problem like poor blood supply to penis or a broken artery or vein.

please advise what i should i do.i refuse to take any erection medication. i'm too young for that and unlike what doctors say...it wont boost my confiedence being on an old mans drug at 27.

i just want a clear answer as to what is happeneing and how i can fix this permanaetly and without the use of any drug or surgery.

am i asking for too much? im 27 years old and healthy. i should not be having these symptoms. especially for a disease that doesnt have any cure and no good treatment options. all of the treatment options are lousy and embarrassing.

Brief Answer:
I am sorry to hear about your troubles

Detailed Answer:
Hi XXXXXXX young1888,

First of all, I am sorry to hear that you do not feel you are being heard by health care professionals.
Second, the observation that you in fact CAN get erections argues against significant physical illness. However, I would expect a standard physical examination including blood pressure, ECG and tests for diabetes before concluding that it is "anxiety". On that note, have you experienced anything disturbing or upsetting in your life during the last year or so? Troublesome relationships, etc.?

Looking forward to hearing from you.

I got blood work done and everything came back normal. When I said it may be a physical issue I meant more of artery or vein damage.

Like what would be the cause of not sustaining an erection during masturbation? It’s like when I take my hand off my penis for like 15-20 seconds I feel my erection going down.

No, I have not been in a relationship this past year so I do not have relationship issues.

This happened around the time I found out I had phimosis and I guess I was pretty sad about that. This issue actually happened maybe a week after my phimosis diagnosis and I was using a steroid cream to help retract the foreskin. Would that cream have caused internal penile damage with the veins or arteries?

I guess the main physical problem that still hasn’t been ruled out is venous leak right? Since people with venous leak can still get erections but they lose it fast?

Brief Answer:
Venous leak unlikely

Detailed Answer:
Hi again XXXXXXX young1888,

It is unlikely that the steroid cream has affected the skin permanently. A venous leak is also unlikely, but this should be able to diagnose on either ultrasound or a specific CT angiogram. You should here you GP about the possibilities of such scans.

Hope this helps. Please let me know if you have further questions.

how unlikely is a venous leak and what are the symptoms?

I'm basically trying to rule out physical causes.

does psychological ED effect all aspects of erections including during sex, masturbation, morning erections, etc.?

Brief Answer:
It can be due to venous leak

Detailed Answer:
Hi again,

Your symptoms are actually suggestive of a psychological cause and venous leak as the symptoms are similar. What you should do is get a penile ultrasound and a cavernosography as this is often necessary to diagnose the venous leak.

The condition is underdiagnosed so it is difficult to determine the likelihood of a venous leak. Treatment is often surgical ligation of the leaking veins or a penile implant.

Let me know if you have further questions.

but would somebody with a venous leak ever get erections? and if so how long would they last? and also what are the causes of venous leak? i am not really wanting to get the doppler test done unless it is 100% required. i just think it is too invasive and i dont like how the doctors has to inject the penis with something.

and if you think my symptoms are suggestive of a psychological cause then why would i need to get the doppler test done?

also is there no other way to naturally cure a venous leak and restore erectile function? why is the only options to get surgery or to get an implant. i wouldnt want to do either one of those things. is there anything else that can be done to restore erectile function?

Brief Answer:
Erections are still possible with a venous leak

Detailed Answer:
Hi again XXXXXXX

With a venous leak, you still get erections, but they are somewhat flaccid and you lose them relatively quickly. No one really knows what causes venous leaks. The Doppler and cavernosography should be done to rule out a physiological cause, so that if these tests are normal, you could consider seeing a psychologist for counselling.
To my knowledge, there is no natural cure for a venous leak. It is all about reducing the venous flow away from the penis, so in theory, a penis ring that is applied at the root of the penis could help. But be aware that it must be possible to immediately take of the ring in case the penis starts to hurt.

Looking forward to hearing your thoughts on this.

and you're saying my symtoms are suggestive of a venous leak?

i just have a few questions.
with a venous leak wouldn't it be a more gradual onset moreso than a sudden onset like mine?

Also, I would say my erections are pretty solid when I get erections now. And when I am physically with somebody my erection stays the entire time (not through intercourse but just making out/kissing naked).

when I say my erections decrease quicker I mean when I have an erection and am no longer aroused it tends to go down quicker than normal.

also, would morning erections still be possible with venous leak? if blood just seeps put quick then wouldn't morning erections be impossible?

i also hear with venous leak ou cant get an erection or keep an erection while standing up which i still can. is this true?

i'm sorry in advance and i know this is not your fault but is there seriously no cure for a VL? Am i really expected to just wear a cock ring until i die? I'm only 27 years old..I would like to just reverse these symptoms and have normal erections again.
the only other option i have is to get an implant which i dont want at all. Isn't there just a way to fix the leaking vein? be it through medicine or something?
I'm not saying I 100% have a VL but I thought you said it is unlikely. and what would be the point of doing the doppler test if the only options for VL are just to get an implant? its not like being diagnosed with VL is going to have any benefit to what is currently happening..

VL is treatable

Detailed Answer:
Hi again XXXXXXX

I find that your symptoms are suggestive of VL, but they are difficult to distinguish from a psychological cause. The symptoms do not necessarily have to start gradually, but the fact that your problems differ from different situations points more to a psychological condition.
Morning erections will still be possible with VL as will standing erections.

I completely understand your distress. I think it would help to have an ultrasound (and possibly cavernosography) and if they show nothing is wrong you could always consult a psychologist.

How do you feel about that plan?

I don't like the plan. I want it to be cured. I don't want to get an implant or wear a penis ring at 27 years old. How hard cold it be to just have it be completely cured?

How long do I need to have an erection in order to rule out a venous leak?

Nothing in this situation seems ideal. Even the testing is invasive with the needle.

It just seems like a lose, lose situation. I mean I don't know anybody who would be okay with having a penile implant or having to wear a penile ring every time they want an erection.

Is there seriously not one thing that's available in medical science that could permanently cure this? If not, then the what is the point of getting any further testing done? What would I possibly accomplish by getting tests done if I am not liking any of the available treatments? This has been such a huge issue for me the past 9 months that if I do end up getting diagnosed with a physical ED problem at 27 I will become so depressed and suicidal I don't even want to think about it

Unfortunately there is no natural cure

Detailed Answer:
Hi again XXXXXXX

Instead of going for ultrasound and cavernosography first, you could try it the other way around by starting with simple cognitive behavioral therapy with a psychologist to see if this fixes your problems. If it doesn't help, you could opt for ultrasound and further imaging.

Does this seem like a better approach to you?

You need further testing to be sure it is VL

Detailed Answer:
Hi again XXXXXXX

As I mentioned, it is not certain the you in fact have VL; you will first need to be examined to be sure. The fact that you can maintain (morning) erections argues against VL and points to what might be a psychological cause, thus underscoring the idea of cognitive therapy. This therapy has in fact been shown to help some of the problems associated with erectile dysfunction. Whether it will help you or not, is obviously not given.

how is cognitive therapy gonna help?

the best thing that would help is if i can get the same erections and libido as i did before this entire mess started. so there really is nothing that can be done if it is a venous leak to restore erectile function? that's really depressing and sad..

also can you explain how morning erections are still possible with a venous leak? because i thought a venous leak meant your erections go down fast so how would keeping an erection the entire night be possible? also if you havea venous leak you would have position dependent erections as well right? you wouldnt get an erection in certain positions.

is the therapy just switching your mind away from thinking about erections and erectile dysfunction? I have been thinking about that 24/7 for 9 months now.

I've had hypochondriac moments in my life before such as thinking for sure I have HIV, herpes, appendicitis, infertility, etc. and I always got tests done and the tests showed nothing was wrong. I think the reason I am still stuck on this erectile dysfunction hypochondria period is because no doctor has done a test outside of a blood hormone test yet so I am not 100% reassured and the constant worrying and anxiety about my erections is probably irritating my erections more.

Also, do you happen to know what the disease or condition the body sensation I felt back in March may be? Or linked to? The feeling of my pelvic area feeling weak and detached from my body was what triggered my hysteria into thinking I had a penile/erection problem. Does that mean I have a weak pelvic area? I googled symptoms of this and i can still control my bladder and bowel movements but I can't think of any other reason why it would feel numb/weak like that where I couldn't wiggle my penis.

Very astute observations

Detailed Answer:
Hi XXXXXXX

Those are some very observations you have made, and I certainly agree that there is an element of a vicious cycle, where your constant worrying worsens your erectile dysfunction, which aggravates your worrying, and so on. You mention that you could be reassured if a doctor did something outside of a blood hormone test. This underscores my previous suggestion of an ultrasound of your penis.
Cognitive therapy is a way of helping your mind breaking out of the vicious cycle as mentioned above, and it is definitely worth a try imo.

From what you describe, I do not think that you have a weak pelvic area.

Looking forward to hearing your thoughts on this.
