Possible causes and further needed steps explained below

Detailed Answer:
Hello and welcome to Ask a Doctor service,

I carefully read your query. I am sorry for the struggles you are having.

Pain with movement is generally muscular in nature. There are other causes as well. If your pain is mostly on the chest area, COPD may be the cause. COPD may cause shortness of breath as well.
As for the pain in the kidney area, it may be due to the kidney, or lungs. You will need a urinalysis to make sure your kidneys are fine. An abdominal ultrasound and abdominal and chest x-ray will be important to rule out kidney stones and pleuritis (inflammation of the thin layers covering the lungs, which may cause pain in the chest or kidney area).

You are taking an opioid drug. This may cause shortness of breath and respiratory depression. Generally, caution should be when using it in COPD as it may cause serious respiratory problems.

To conclude:
- there is a wide range of conditions that may cause torso pain. Very often it is related to muscles, especially if it is worse on movements
- you will need an abdominal ultrasound, abdominal x-ray, chest x-ray to make sure your kidneys are OK (no infection or stones), there is no pleuritis, and liver and other abdominal organs are fine.
- ALT, AST, Bilirubine, may need to be checked as in rare cases buprenorphine (belbucca) may cause severe liver problems
- you will need to talk with your doctor on the safety of buprenorphine while you have COPD, it may worsen the shortness of breath

I hope this answers your query. I remain at your disposal in case further medical assistance is needed.
