Question: Woke up this morning felt a sharp pricking pain in the epigastral area. It was very brief lasting a few seconds. This is the second time this happened upon waking this week. I had an endoscopy two years ago and was diagnosed with GERD could it be related

GERD is controllable but not curable therefore yes it could be a flare up of episode of GERD.
I would suggest you to take any antacid like Prizolec for 5 days and if the pain responds to that proper treatment for acid peptic problem has to be taken.

I am currently taking 20 mg of Prilosec a day do I need to change medication or could this be breakthrough reflux? Also concerned could this be an ulcer
