Will Missing A Single Dosage Of Isoniazid Lead To MDR TB?

Question: Hi Doc, My husband is diagnosed with potts Spine. He had been advise to take akt4 daily. He has been taking the medicines regularly without fail since 2 months. Doc suggested the sequence of dosage as 1 capsule(1 (cap) - rifampicin 450 mg) in morning empty stomach, 1 tab after breakfast (tabs) - pyrazinamide 375 mg , 1 tab after breakfast (tabs) - pyrazinamide 375 mgafter lunch and last tab after dinner( Ethambutol Hydrochloride 800 mg + isoniazid 300 mg). By mistake he missed last tab yesterday night which was supposed to be taken after dinner. In morning he remember and due to panic he took that missed tab in morning empty stomach along with rifampicin. I wanted to know will he develop drug resistance TB due to SINGLE dose missed. Approximately how many doses needs to be missed to develop MDR TB. He is quite scared that he might have develop the resistance to medicines since he missed single dose. Please advise urgently. First of all ask your husband to calm down . Missing one single dose will neither affect the outcome of treatment nor will lead to drug resistance.

There is nothing like certain amount of doses that has to be missed to cause drug resistance . In fact even if TB medicines are missed for months it may not lead to MDR TB ( but it definitely affects the cure of the disease ) . But as an answer to your question if the TB medicines are stopped for more than 1-2 months or if not taken regularly or if the dose is not adequate then the chance of MDR TB increases.

So just relax and let him take AKT4 as usual.
