Question: From past 2-3 days, i have continuous mild headache and mild body pain. Also for some period in between the day i feel cold as well. Today i get my blood test done. Lab Attendant said the according to report i am diagnosed with Malaria.
I am attaching the report. Kindly check and suggest.

Yes, the report indeed shows positive results for malaria (Plasmodium vivax). But it would be worthwhile to get the test repeated at any NABL certified laboratory - both MP smear and rapid antigen test.

If repeat tests is also positive, then it can be treated with specific antimalarial drugs - chloroquine + primaquine. Your local doctor will know about its treatment.
