Thanks for query ,

Based on the facts that you have posted you seem to had UTI for which you were being treated with antibiotic .Your serial urine analysis have revealed decrease in pus cells in urine however the recent report suggests presence of 12-15 pus cells suggestive of either infection not being eradicated completely or recurrence of infection

I shall be happy if you can post me more details to help me in making precise comments
1) Do you have any obstructive voiding symptoms like frequency for urination ,nocturia,urgency ,burning etc ?
2) Was your urine culture done if yes please mention the name of antibiotic that was being prescribed ?
3)Are you diabetic ?

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.

Further treatment will depend upon result of these tests and final diagnosis.

Dear Dr. XXXXXXX

Thanks for your mail. To answer questions asked precisely;

(1) In Sept'16 diagnosed BPH thereafter taking urimax d every night; able to empty out bladder in 1 min

(2) Taking 3.4 to 4 litres of water daily

(3) Border line diabetic range last Hb1Ac = 6

(4) today given urine for culture.

(5) Will do PSA and Ultrasound as prescribed

Any further advise please
