Question: myocardial perfusion imagine reversible perfusion defect mild to mod intensity in territpry of RCA, scar/ infacrt noted in basilar inferior segment--hypkinesis of inferior wll. My E/F is 56% Overall function left ventricular systolic function is normal with hypokinesis of the interior wall. What does this all mean. Iam 74 year old male my weight is 198lbs.

Hello!

Welcome back on HCM!

Regarding your concern, I would like to explain that a mild to moderate intensity of reversible perfusion defect in myocardial perfusion scan means that potential mild coronary lesions may be present in the right coronary artery (RCA).

There may be non-important coronary stenoses in the right sided coronary artery, which supplies a territory of the heart where a previous myocardial infarct has happened.

In other words, some myocardial territories surrounding the previous infarction suffer in a subclinical (silent) manner form mildly insufficient blood supply.

From the other side, hypokinesis of the inferior wall is a consequence of the previous myocardial infarction (which has left permanent cardiac muscle impairment).

The good and most important issue of all the above information is the fact your overall cardiac function is preserved (within normal values).

Cardiac coronary catheterization would reveal the extent of these mild and potentially non-important coronary lesions.

Hope to have been helpful to you!

Kind regards,

Dr. Iliri
