What Does The Following Widal Test Report Indicate?

Question: Widal test shows, do have typhoid ?
TYPHI O 1/20, 1/40
TYPHI H 1/20, 1/40, 1/80
PARATYPHI A(H) 1/20, 1/40, 1/80
PARATYPHI B(H) all negative
No fever, no body pain, only have cough from last 7 days. cough coming out moderately.

I've gone through your query and understand your concern,

I've seen the attached report, and as per this Widal report, it's inconclusive whether he has Typhoid or not.. Although the titers are not significantly raised which goes in favor of not having typhoid, still confirm diagnosis can only be made after blood culture and tyhoid specific antibody IgM and IgG (if symptomatic, but as per your descriptions those symptoms are not there)..

You should also note that Widal is not a very sensitive test to diagnose typhoid/paratyphoid infection..

Since there's only cough, and no fever/body pain (is there any pain abdomen/loose motion ? , since you haven't mentioned so I'm assuming it's not there too), then most likely it's not typhoid..

He should be thoroughly examined by the Internal medicine specialist clinically to look for causes behind the cough (multiple reasons can be there, starting from pharyngitis, allergic manifestations, other upper respiratory tract infections etc.)..

No abdominal pain or loose motion present, rather he is bit constipated. He is having antibiotics for cold and cough. Is there any medicine he needs to take to make sure typhoid does not come back ? What is most likely cause of typhoid infection ?

Well then it's most likely not typhoid, and upper respiratory tract infection only, yes continue the full course of antibiotics as advised by his doctor..

Since it's most likely not there in the first place so you need not worry about it's recurrence..

Typhoid is a bacterial infection which mostly spreads through contaminated water and poor sanitation and sometimes direct contact with the infected person, so these are the areas must be maintained cautiously for future prevention..no outside water strictly (only U.V.R.O./mineral water should be taken)..maintenance of hand hygiene (wash thoroughly with anti-bacterial hand wash before and after using toilet, always wash properly before and after having any foods)..

Family doctor has told him to have "cefixime and ofloxacin" 10 tablets. Is it ok to have ? Currently he is having antibiotics for upper respiratory tract infection. Doctor has advised to have it once the ongoing antibiotic course finishes.

You haven't specified which antibiotics course he's having currently ?
And was it prescribed by the same doctor only ?

The Cefixime and Ofloxacin is typically prescribed for typhoid/parayphoid infection, but your description doesn't pertain to it, so I think it's right time to ask his family doctor, what is it that he's suspecting ? Since he's having an upper hand in this scenario (as he has the exclusive privilege to examine him clinically), so just feel free to ask him which will also give you proper clarity in this regard (as there must be some clinical features,and/or some other lab parameters which is making him suspecting the same)..
