What Does My Ultrasound Scan Report Indicate?

Question: Hi Doctor, I am attaching the ultrasound report of my wife's pelvis which shows that she has a fibroid measuring 4.5 x 3.5 cm and also thickened endometrium measuring 1 cm. She has seen two gynecologists and I've attached their prescriptions as well. I've a few questions regarding this:
1. Can the fibroid of this size be treated with medication or operation is the only option? And what is the medication you suggest?
2. My wife's age is 37.5 years, however we got married just 9 months back and we plan for pregnancy soon. So, can this fibroid be affecting the pregnancy in any way? And should we plan for pregnancy first and then go for medication / operation for fibroid or is it okay to go for medication / operation first and it'll not affect the pregnancy capability?
3. Prescription 1a and 1b were given by first gynecologist while prescription 2 was given by second gynecologist. We are not sure which one my wife should follow. Can you please help me with that?

I understand your problem and confusion related to the issue.
I have seen the ultrasound report, ultrasound picture and prescriptions attached.
This is a transabdominal ultrasound which is not very sensitive in picking these fibroids properly.
I would suggest you to get a transvaginal ultrasound to see the precise size of fibroid and whether it is pressing the endometrial lining (where baby is attached in case of pregnancy).
If endometrial lining is free of fibroid impact then try for pregnancy first and surgery later.
Medicines will not be a good option for you as as you guys are trying for pregnancy.
Get transvaginal ultrasound and then we can discuss further management
