Question: Hi Dr,

I just have a question about phimosis and if I possibly have it. So I am almost 18 and I have the ability to fully pull back my foreskin when I am not erect but when I am erect I can't do it fully but just over half of it. So is this a bad thing, should I get it looked at I mean I never really thought about it at all? Is there ways to stretch it or someting along those lines, I dont have any pains when masterbating or passing urine but it hurts if I try to stretch the foreskin anymore than I can. Also should I be washing the head of my penis with water every day in the shower as hygiene?

Kind Regards, XXXXXXX

Brief Answer:
Yes,one should wash glans penis daily while bathing.

Detailed Answer:
Hi XXXXXXX
Welcome to health care magic,

There can be two possibilities of difficulty in pulling back fore skin.

1) Phimosis (tight skin with small hole) or
2) tight frenulum on under side of penis (a tag of skin or mucosa attaching foreskin to the glans penis).

In both cases, person might have difficulty or pain during masturbating or during sex. This problem is not uncommon in younger males,so it is not related to good thing or bad thing.

In some cases it requires medical attention.

Yes,one should wash glans penis daily while bathing just retracting his foreskin downwards and by rinsing with clean shower water. So be sure to deal with it as part of your daily hygiene.

Cleaning actually removes the excess smegma (which is fluid drops containing of dead skin cells, seminal remnants, urine deposits or sebaceous oil), leaving a healthy, natural, micro thin layer of this beneficial product to keep the glans moist and sensitive.

Never use soap under the foreskin as it leaves soap deposits that reactivate in the natural moisture, and cause burning or irritation.

Soap also removes all the natural moisturising effect of the smegma, leaving the glans and foreskin dry and open to infections.

If both this problem (phimosis) persists even after daily cleaning then circumcision would be the best option for which you can consult general surgeon who will know best about the procedure and can help you.

Hope I have answered your query.

Do you think because I haven't been washing my penis head every day and pulling back my foreskin could be the cause of my tight foreskin? And I would rather not have circumcision because I have heen told it is not needed. So would pulling it back and cleaning it every day help stretch it even if I am flacid?

Brief Answer:
Yes,daily washing glans penis will certainly help.

Detailed Answer:
Hi XXXXXXX
Thanks for the follow-up.

Only in some cases it requires medical attention like circumcision.

Do ,one should wash glans penis daily while bathing just retracting his foreskin downwards and by rinsing with clean shower water.

Cleaning actually removes the excess smegma leaving a healthy, natural, micro thin layer of this beneficial product to keep the glans moist and sensitive.

Therefore, it will help to stretch easily the fore skin back to the glans penis even in flaccid condition.

Hope this will help you.

Thank you for clearing that up, just a couple of last things I have heard that if you pull it too far back it can get stuck. So what would be the best method when cleaning? Should I slowly pull it back as far as I am comfortable with in the shower and should I touch the head while cleaning it or should I just let water run over it? Another thing I noticed is that I can't keep it out for long until the foreskin contracts back to normal. I just want to know the safest technique and what to do if I did get in the situation of it being stuck.

Brief Answer:
See the explanation.

Detailed Answer:
Hi,
Thanks for the follow-up.

I understand what you want to know.

Just retract your foreskin backwards from the glans penis to expose it and gently clean the glans with using index finger in a circular motion over the glans and let the shower water run over it.

This cleaning procedure can be done while bathing frequently to clean the excess smegma and other secretions as I discussed earlier.

Here,do not use soap as it may cause irritation and burning sensation.

In case, if fore skin got stuck, you can gently push the back skin downwards and pull back the skin of glans to cover it again.

Hope this will help you.Regular cleaning will solve your problem,so there is no need to worry.
