What Could Cause A Gradual Weight Loss Despite Following A Healthy Lifestyle Except For Hypertension For Over A Decade?

Question: Hello Doctor,

I want to ask about my Father.

Am living 250 KMS away from my parents, who are in native.



He is 69 years Old. I was working as a laborer then retired as a supervisor for a Steel Plant (SAIL), he got retired at his 60 years.

When he was working 9 years ago he was fit and muscular and weight was around 75 kgs (height - 167 cms).



From past 2 years we are observing his weight is reducing GRADUALLY.



3 months back (in April) we went for a wedding (in a village). After having food their he fall sick with Diarrhea for 1 week. And he has become lean (quick) weight reduced to 50kgs.



Now, his daily routine is - wake up at 7:30 am have Ragi malt, around 9:45 he will have breakfast (Idly/Dosa/Palav/etc..) and he will go Out (walk for around 2kms). Around 1:30pm he will have lunch (most of the times Rice n Sambar) followed by a Nap (1 hour), have Tea around 5 pm and go for a walk for around 5-6 kms. Around 8:30pm he will have Dinner (Chapathi/Ragi Ball and little Rice) and Sleep.



NO Symptoms of - Heavy Sweating, breathing problem, Tiredness.

Medical History - Hight BP from past 15 Years. He takes tabs and it is in control.

NO Diabetes, Cholesterol is Normal. ECG is fine (as per doctor).



Problems:

1. Whenever he have food Outside in hotels within hours he will have Loose Motion (we give him Eldoper tab, Lactobacillus Sporogenes Tab and Citro Soda satchet). He will be fine.

2. Now his Current weight is 50 Kgs.

3. If he travel/don't walk/sit for long hours he will have back pain for 2-3 days.



My Questions:

1. Is he health? is this common for his age?

2. Is he a malnutrition?

3. How can he gain some weight ?

4. He walks for 5-6 kms daily without any issues.

5. Being his Son how can I take care of him and keep happy at his age?

6. Does he require any supplements like eggs or whey protein? Is it advisable to take?



He had struggled a lot to bring me and my brother to this stage. We both are Engineers and earning handful. We want to take care of him at his age.

@ Diarrhea after outside food is normal at this age if the intestine flora disturbances
occur but if it is associated with bleeding or else condition where he gets diarrhea
after home food also then its problematic.

@ Back pain is related to possible osteoporosis and spondylosis involving spine bones
which again is age related ailment.

@ So far as health is concerned, he can be said a semi healthy state as weight is some
what low ( but still recent weight of 50 kg with 75 kg weight of 9 years back , can not
be a comparative data medically ).

@ He can be considered malnourished to some extent as per the possible muscle
wasting, bone weakness.

@ To make him gain weight, he must get balanced nutritious diet with all vitamins,
minerals, protein, carbohydrates, iron, trace elements, fat and so forth.
So his current diet is very unbalanced, which require so much modifications as
He requires green leafy vegetables, cereals, pulses, legumes, milk, organic fresh
fruits, mung ( sprouted ), honey, jaggery etc.

He must be given supplemental vitamins, iron, protein in terms of tablets, syrup or
powder form to support the general debility.

@ At this stage, to keep him happy, the best thing you can provide is your time from
highly busy professional career.
You can notice the charm on his face and health difference if you regularly stay with
him on weekends and as you are 2 brothers, you can manage easily on alternate
basis.

@ Eggs can be given to him with above said ailments, whey protein not recommended
without clinical assessment.

@ His weight chart should be maintained on weekly basis along with control of BP and
blood sugar.

Sorry doctor there is a typo (my bad).
His current weight is 60 Kgs. I confirmed with him.
Also he mentioned, he was 69 kgs 3 months back. Now he is 60kgs.

Should I consult a doctor for malnourished (Yes, Muscle wasting has happened a lot). Can you refer good doctors if you know?

Can he re-gain the weight and become healthy again?

* Yes, 9 kg weight loss is significant for his age in a period of 3 months.

* I recommend you to consult him an expert physician in your city ( frankly speaking, I
do not know the same at your place ) and get basic evaluation with blood tests, x-ray
chest and ultrasound of the abdomen & pelvis ; through which doctor will be able to
convey you the cause of weight loss.

* Do not get disturbed from right now, once diagnosis is made, after medications he will
be gaining back his rhythm but such things take time and one has to keep patience.
