What Could Be The Reason For Recurring Fever In A Child?

Question: Hi Doctor,
My daughter is 14 months old and her weight was approx. 10 kg one month back.
She had fever on 3rd March (after playing with water on Holi on 2nd March) and developed cough and runny nose as well on 4th March .

We were giving her following based on similar condition approx. 20 days back.
1. Calpol every 5-6 hours 1.5 ml
2. Alerid 2.5 ml twice a day
3. Maxtra 2.5 ml thrice or twice a day

We did that for 3-4 days and fever was not there, after that we stopped Maxtra and Calpol and just continued with Alerid for 2-3 days more.
But, after that she was again having 102 F fever yesterday (12th March) and we are again giving her calpol and Alerid in the same dosage as mentioned above.

Please suggest if you can see any specific reason of fever coming back after 4-5 days. As of now she is having
- Fever
- Runny nose
- Chest congestion

Please suggest what shall we do in this case. Thanks in advance for your help.

Possibly due to viral cause. Irrespective of treatment may last for 7 days.

Detailed Answer:

Hi,

Most often such episodes are due to viral infections. The mentioned medicines were proper and you need to continue them in the same dose since the fever is back. Also, give tepid sponging.

As I have mentioned, such fevers are usually due to viral cause. Usually, they are associated with an allergic component (which is responsible for the running nose and for which you are giving Alerid or Maxtra). Steam inhalation would help in relieving the chest congestion. Just sit with the child on your lap and place the source of steam before you. The water vapor will be in the air and automatically the child will breathe it in.

Please be patient. It should be ok back again in a few days.

Hi Doctor,

- This viral fever generally lasts for how many days?
- Is it normal that the viral fever comes back in 10-12 days as it happened with my daughter?
- Is it safe to give calpol for so many days? 3-4 days we had given from 3rd March and again we are giving her since 12th March as and when needed?
- We are giving her Arbivit 3 forte drops 0.5 ml and Visyneral zinc drops 1 ml everyday as per the doctor. Shall we also give her something else to increase or build her immunity? as due to this viral fever she looks dull and weak. Please suggest.

Typhoid might need to be ruled out in case of prolonged fever.

Hi,

A viral fever usually lasts for 7 days or slightly longer. There is a possibility of recurrence but in case of fever lasts longer it would be worthwhile to get typhoid fever ruled out. It is safe to give Calpol even if the fever occurs recurrently or for prolonged durations. Please continue to give Arbivit Forte and Visyneral Zinc Drops. Give colourful fruits and vegetables for the enhancement of immunity. Medicines will not help. It is not unlikely for the child to look dull and weak after such fevers. You need to be patient and give wholesome food.

Hi Doctor,
One of my friend's child is also having similar kind of fever and is given Meftal-P by the doctor saying that Meftal-P is better than calpol/crocin in fever above 101 or so.

Any suggestions regarding this whether Calpol or Meftal-P should be given? As said in earlier query, we are giving calpol as of now (when needed).

Yes, Meftal P is more effective than Calpol (Paracetamol). The primary significant adverse effect is the gastric irritation it causes. You can feel free to use Meftal P if you wish. Paracetamol, on the other hand, is a very safe drug.

Hi Doctor,
Its been 7th day today since she is having fever. Previously we had to give calpol every 6 hours now, we have to give it every 8-10 hours.
Also, her cough seems to be increased a bit today and runny nose seems to be more of blocked nose with the sound while she is sleeping.

We are giving Calpol (SOS), Alerid and Ambrolite S as of now. Do you feel we should give some antibiotic as today is almost 7th Day with no improvement in her condition as such. If yes, in what dosage? Please suggest.

Get a few blood tests done.

Hi,

Giving antibiotics randomly will not help. The cause needs to be ascertained. You can get a blood count, CRP and Widal test done. Let me know the reports and I will guide you accordingly.

Since I am not suggesting an antibiotic as of now, the dosage does not apply.
