Question: I'm a 38 year old male. I found a couple droplets of blood in the front of my boxers when I woke up the other morning. I have not noticed any discoloring of my urine nor semen. And it has not happened since. I feel fine for the most part otherwise. I'm getting very nervous....

Presence of droplets of blood in the front of boxers or underwear may be due,

1. Irritation/ Inflammation of testicular vessels or the urinary tract
2. Trauma to the testicles
3. Sexually transmitted disease (STD)
4. Inflammations/ Infections of the genital tract (vas deferens, prostate or seminal vesicles)
5. Prostate cancer (very rare).

You should go for certain Investigations like urine examination, cystoscopy and an ultrasound scan of the pelvic & testicular area will be helpful in clinching the diagnosis.

So,it will be better to consult an Urologist to find out the exact cause,So that further important steps could be taken.

Thank you for your insight. I realize it's difficult to speculate through virtual/email without seeing a patient in person. I'm fairly certain it's not STD, and I do not recall any traumatic occurrence to that region. I'm 6' tall, 175 lbs. I'm in fairly decent shape, although my exercise frequency has a lot to be desired. I'm a regular/heavy drinker, could alcohol play a role in this( I read another post about that being a possible cause)?

Hi,
Thanks for the follow-up,

I have mentioned possible causes although they are uncommon.

One possibility can be your daily work outs where occasionally blood or its droplets might come out in urine but alcohol has no role. However I would order for urine tests and ultrasound scan abdomen-pelvis and testicular area to rule out infection/inflammatory condition this problem persists.

In the interim, please drink plenty of fluids and observe.
