What Causes Spotting, Painful Intercourse And Sore Breasts?

Question: I have been on Jolessa birth control for about 6-8 months now. I used to take my pill everyday at the same time (10:00am) . I got married four months ago and my husband and I have intercourse everyday. For the last month or so I've been extremely bad at when I take my pill. I always take it everyday but a couple of times it wasn't until night time. I took a negative pregnancy test a week or two ago. My breasts have been sore, and this week I had light spotting for two days (which I've not had in the 6 months of taking this birthcontrol) after the two days of spotting intercourse was very painful which is also not usual for me. Is there any chance I am pregnant? I considered a cyst but I had a pelvic exam recently.

Thanks for contacting HCM,

It is understood that you have been on jolessa oral contraceptive pills on a regular basis since last 6-8 months which has prevented ovulation completely and offered you protection from pregnancy.
If after marriage, since 1 month, you have been irregular in taking the pills at the same time and the gap has been more than 6 hrs . As you have been involved in regular daily sex with your partner , the concern is that whether ovulation was prevented during this cycle or not. If the pills were regularly used in the initial 14 days of the cycle then it would have prevented any ovulation , but if you are in the mid-cycle at present and you had spotting for 2 days which is indeed a withdrawal bleed due to irregular intake of pills, chances are that ovulation may occur.

Performing a urine pregnancy test in the mid cycle would not be sensitive at all .
It is necessary to know your LMP ( last menstrual period for this cycle) and you are on which pill day today?

It is suggested that you practice additional barrier contraception along with the pills to ascertain pregnancy protection for this cycle as ovulation is uncertain.

The best way to rule out pregnancy would be to do a serum beta hcg test on day 21 of the cycle, if ovulation is suspected and pregnancy is contemplated by chance. This hcg levels if found raised or more than 1 shall confirm pregnancy else rule it out if less than 1 and you can safely continue pills fr the cycle with additional back up method.
