Question: my wife has a terrible problem sleeping at night, she takes melatolin and over the counter sleeping pills and it just doesn't help.... my thought is that if she had something to help her sleep she would get in a pattern of sleeping better and then wouldn't need this down the road

Brief Answer:
Medications and counselling will help her.

Detailed Answer:
I can understand your wife's situation and problem about the altered sleep rhythm. This is very common problem that I see now a days. The common cause that I see with most patients with similar problem is stress and tension. If she is has stressful life then ask her to avoid stress and worries. Similarly beyond 45 years, when menopause is around, sleep disorder can occur due to hormonal imbalance.

So my suggestion for her is
1. Consult psychiatrist and consider counseling sessions. Try to identify stressor in her life and start working on its solution. Sometimes only counselling is more than enough to normalize sleep pattern.
Newer drugs like zolpidem, alprazolam, clonazepam etc are good for disturbed sleep pattern. But you need psychiatrist 's prescription for them.
2. Get done hormone levels and if they are altered then consult gynecologist to start appropriate hormone replacement therapy (HRT).

Hope I have solved your query.
I will be happy to help you further.
Wishing good health to your wife.
Thanks.
