What Causes Recurrent Inflamed Itchy Sores On My Chin?

Question: I keep getting large inflamed sores on my chin that won't heal and itch horribly (unlike acne, there is no single white head). It will ooze fluid and scab over with a scab that is 3/8th of an inch and when the scab peels off it feels like a burn, very raw pain. Then it repeats (itching, oozing, scab, peeling, soreness).

I have a goatee. I don't know if that makes any difference. At time I get this crawly feeling and the sores and the skin around it starts to itch really bad.

My dermatologist took a biopsy to test for cancer (I did have a small basal cell carcinoma behind my ear, though it was just a bump. The symptoms were not the same). It came back negative.

I was prescribed Doxycycline Hyclate 100mg for 10 days. I took and finished the prescription and it had no noticeable affect. I also was applying Mupirocin 2% ointment, again, it had no affect and seemed to aggravate the sores.

My dermatologist injected the larger sores with Cortisone which did cause them to shrink but a new ones appeared. I think the shots treat the symptoms (inflammation) but not the underlying cause.

Recently I've been on Monocycline 100mg once a day and I've been applying Cloderm cream twice a day. Nothing seems to be working and the sores are spreading and getting bigger.

Could this be a fungal infection? If its bacterial, is there a stronger anti biotic that I should be taking?

Such itchy sore recurrent bumps on chin in males is most commonly due to a condition known as sycosis barbae. It is basically a bacterial infection found in males in beard area. In such cases, I prefer to put my patients on tablet dapsone after G6 PD assay to rule out any concontraindication.

Also topical creams like mupirocin or sometimes in combination with steroidal cream is advised.

But before this, I will recommend you to put a pic of the lesions so that I can actually visualise them and help you to my best.
