What Causes Raised Blisters On Upper Left Shoulder And Back?

Question: I have what appear to be scratches on mu upper left shoulder, going from the top of shoulder to the middle of my back, almost my waist. I could not have made them I can't reach, and just realized that the scratches are very raised blisters. I have multiple health issues including an ICD, diabetes, lyme, and asthma and wonder if I should go to the er. I have been hospitalized for these other issues 5 times in the last 14 months, and don't want to go for nothing, but this is a large area.
