What Causes Purple Swollen Area All Around Rectum Post Anal Sex?

Question: Hi there. Forgive me if I offend anyone. I have purple and swollen skin all around my rectum following receptive anal sex. Initially it was only one hard lump but following more sex it seems to now have spread further around my rectum- although it doesn't feel as hard in the new areas. After the first cyst appeared I waited some time until it was almost gone before I tried again. But immediately after the second time it spread. I guess I didn't wait long enough before letting the spot get irritated again. There is also an ulcer looking cut at the top section. Yikes. Is this something to be concerned about?

I have gone through your query and picture. It looks like an anal fissure. Avoid anal intercourse as it can worsen the condition.

Initially this can be treated conservatively. You can take stool softeners like softovac. Consume a high fibre diet.

Sitz bath will give relief from pain and swelling. Gels or ointments containing anesthetic and corticosteroid can be used. Ointments like proctosedyl you can get over the counter for this.

Antibiotic also will be needed. I would recommend fluoroquinolone like ofloxacin and ornidazole combination for this. For this you can consult your GP.

If not responding to conservative measures then surgical intervention will be needed.

Thanks doctor XXXXXXX Just in relation to the purple ring of swelling that extends all around the rectum, is that just bruising or a hemmerhoid or something like that ? It looks bad so I'm concerned it could be something serious.

The swelling and redness is due to inflammation. In hemorrhoids also these occurs. I see a fissure there. This can come as a complication of hemorrhoid or trauma. As you had anal intercourse it is most likely from that. As the treatment is same you can go with the above mentioned measures. In case there is increasing pain and discomfort even after conservative measures then you should be concerned.

In case there is no improvement get it examined by a general surgeon. You might need a digital examination or speculum examination in such case.

Thanks again. Can I ask if it is possible to initially avoid using anti biotics keeping the area as clean as possible in the hope that infection does not arise? Are there any alternatives to antibiotics? Hoping to avoid them.

Antibiotics you can avoid initially. Sits bath will be helpful to some extent. You can use aloe vera gel which also has antibacterial action. This you can get as over the counter.

But in case there is increasing of pain and discomfort you should approach your doctor for antibiotics.
