What Causes Persistent Cold And Cough Along With Chest Congestion In A Child?

Question: Hi Doctor,
My 2.5 year daughter has cold and cough and congestion recurring since 5-6 months now.Consulted couple doctors for the same and one of the doctor adviced for couple of tests one of the test immunoglobulin ige serum values are 181 and the skin test for TB is normal (Negative).Kinldy advice is this normal or something to do with asthma or something to worry ??

By what you quote I feel what your kid could be having viral associated wheeze or multi triggered wheeze. I have a few questions for you -
Questions:
1. How many days per month does she cough or feel breathless?
2. How many nights per month does her sleep get disturbed due to above symptoms?
3. Does she feel breathless when she runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?
If your answer is yes for any of the above questions, your kid might be having viral associated wheezing or multi triggered wheezing.

Cough around 10 days per month

15-20 days per month.
3. Does she feel breathless when she runs around or plays with other kids?
Off late she kind of takes longer breathes when she runs around.
4. Are the symptoms when there are seasonal changes?
Yes.
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
Nope.
6. Is the cough always associated with fever?
No.

History is suggestive of multi triggered wheeze

Detailed Answer:
Hi XXXXXXX ..This history what I have obtained after going through the answers you have provided - I feel that your kid has got a multi triggered wheeze. Any kid less than 5 years is not to be labelled as asthma, as there is a fair chance that the kid might grow out of it. So we should not be labeling them as asthmatics.

Another issue is that - seasonal changes, exercise and lower ambient temperatures at night seem to trigger this sort of cough is her and this occurs due to narrowing of the airways which manifests as cough.

If I were your treating pediatrician, I would prefer to start her on Budesonide (Budecort) 100mcg metered dose inhaler 2 puffs in the morning and 2 puffs in the night regularly for 8 weeks along with oral Monteleukast 4mg once a day at 6PM daily (also for 8 weeks) and reassess. These are for preventing further attacks and episodes. But in-spite of these if the symptoms occur, then I would advice Levosalbutamol (Levolin) or Salbutamol (Asthalin) metered dose inhaler to be used only if she is having severe cough or breathlessness. This is the currently recommended therapy for your daughter according to national and international standard recommendations.

If this is what you have mentioned in your query under the heading of - Current medications - then be rest assured, you are in the right direction.

A very important point is that the technique of giving the metered dose inhaler using a spacer and regularity are as important as the medicine itself.

In this sort the therapy if the technique of drug delivery with spacer is incorrect or if the medicine is not given regularly up to the period prescribed by the physician, it might not work. This is what I meant when I quoted above as I will reassess after 8 weeks of therapy. After 8 weeks you need to get back to your doctor and he or she will reassess the patient control of symptoms and modify or extend the therapy.

Thanks for your quick response .another we have noticed is after she joined the school she has these episodes is this is related to school environment & kids around?

Not related to kids- its not contagious

Hi...it is not related to the kids at school - it is not contagious. But it might be related to certain pollen or plants in the school premises or the vehicular smoke and pollution she is getting exposed to during the travel to school and also the inside environment of the class room.
