Question: Hi, my boyfriend has swollen testicles with no pain, what is the cause? The one on the right is pushing
the one on the left.

Hi,

Welcome to HealthcareMagic.

Painless swelling of testis or rather scrotum at his age could be mostly due to accumulation of fluid in the sac, called as hydrocoele.

But this need confirmation by clinical evaluation as it has certain characteristics signs to help in its diagnosis.

So i would suggest that you show him to a surgeon for the same. And if that comes out to be hydrocoele only then it can be treated surgically by a minor operation.
