What Causes Painful Joints, Loss Of Appetite And Sleeplessness?

Question: Hello, Dr. XXXXXXX You answered my last question, so to be up to date, I will direct it to you. You are free to look them up, I am the patient with Advanced atrophy of l. rectus abdominis after T12 Vertebrectomy 11/12. I am not a candidate For Bariatric Surgery having been refused by several programs. Mayo Clinic does handle Complex Cases where surgery is needed to repair problems with previous Bariatric Surgeries & also those with complex medical histories & that is now me, but only since tripping over phone cord in Nurse' Stations as RN. I am beginning to notice other changes-- some gradual, but in last few days some visible to the eye. I am loss here-as I think some may need your service as so far all of you seem to know what do in a few minutes what five or six Drs can't get since early 2013 when first S & S appeared. Your first Dr. got the logical answer in minutes with same set of S&S. Your Marketing Dept. may want to Target Market some Doctors, Enough. I am seeing some strange changes in hands & feet, cramping, drawing up, almost deforming overnight. Cold, with arthritic pain, which I was disabled for, but never noticed, but have certainly heard about over the years. They are painful & other joints as well. Is this an Emergency? I will alert my PCP & Plastic Surgeon tomorrow but reluctant, believe me, based on experience, to go to any ER. Next time I fx a wrist, I'll wrap it myself and see if it doesn't mend itself. I am frightened by sudden changes like this, though, no appetite, sleep, gradually changed in vision, taste etc. I know this is not your field, but I rely on you, & have a Orthopedic Surgeon who I like so will let him know, also. Can you assess my history? I can't seem to be able to actually grip this thing much longer? Thank you, Doctor.

Brief Answer:
You may need anti-anxiety medicines.

Detailed Answer:
Hello
Sorry for replying late

I am aware of your chronic illness.
Your problems may be related to long illness and this is common.
Feeling low,no appetite etc may be due to psychological component.
I suggest for detail evaluation by a psychiatrist. You may need psychological counselling.Believe me,this is very helpful in many chronic cases.
You may need some medicines for anxiety.

For bony pain, you may take NSAID's like acelofenac tablet etc. These are first line drugs in any type of arthritis.
You may need clinical evaluation and investigations related to arthritis. You may need routine hemogram, random blood sugar,RA factor,HsCRP,anti-dsDNA,anti CCP etc.
You may need few medicines after evaluation.

Try to be cheerful and try to keep yourself busy like by listening soft music etc.Though it is very hard, try to overcome the situation slowly.

Get well soon.
Hope I have answered your question.
Further queries are most welcome.

Take Care
