What Causes Pain During Urination Post Frenuloplasty?

Question: Hi Sir,

I had problem of tight frenulum but i was still able to pull my foreskin back easily while flaccid and with bit discomfort while erect.
So, i had a frenuloplasty 14 days ago.
After procedure my my foreskin was retracted and bandaged in retracted postion for 48 hours, which caused lot of swelling.
After having chymoral forte tab for 4 days and then having combiflam for 3 days(suggested by second doctor i consulted), Swelling is gone completely.

But as swelling subsided, i noticed one more issue- There is a white spot under tip of my foreskin(above glan/head of penis).This place is 8mm long and 5 mm wide.
It's hard from outside(hardness has reduced with subsided swelling) and soft, white inside. It causes burning sometimes while urinating.
I applied Tbact cream for few days but it didn't go then i applied Momate cream for 2 days. It started to burn in urinating, so i switched to tbact again.
It's not expanding in area but not subsiding also. I am sure it happened due to excessive swelling and stress caused to foreskin while it was bandaged rectracted.
When i try to retract my foreskin, this area comes to stress and hurts and feels like if i try further, i will end up tearing my foreskin from this scar place.
Unable to retract, i pour some lukeworm/saline water twice a day by pulling foreskin up and apply tbact wherever i can reach.

I sometimes have very small bleeding from my surgery wound(it used to be very heavy in first week after surgery now it has significantly reduced and no blood spots are found on my underwear. Just tip is red sometimes due to small bleeding).

When i asked my surgeon about this, he said that surgery wound may/will not heal if skin is not retracted and wound is cleaned daily. And this scar is difficult to treat(because urinating will not let it heal). So, i will need a partial circumcision to get rid of this problem.
But i don't want to go for circumcision because i am sure that once this scar(above glan) is treated, i will be able to retract my foreskin as before surgery.

Questions:-

1. Will surgery wound not heal if i can't retract my foreskin? Is this small amount of blood normal till 14 days?
2. Is it really impossible to treat the scar (above glan/head and inside tip of foreskin)? i think its a normal scar due to excessive swelling stress.

Please suggest me some medications that can help me, treating the scar because this is the thing troubling me most.
and yes, my wound is not infected yet(may be because i am applying tbact with q-tip and having Augmentin 625 from last 5 days twice daily).

Thanks for your query,based on the facts and photographs that you have posted it appears that you had a band of adhesion between frenulum and glans along with partial Phimosis that was making retraction of foreskin painful and difficult on erection.

Ideally in such situation doing only Frenuloplasty does not serve the purpose .It is always advisable to get circumcision done so that one is able to have enjoyable sexual life in future.

Looking at the sequence of events that has happened there was a swelling of foreskin which was kept in retracted position for 2 days and you developed what is called as Paraphimosis. due to constricting ring at the level of coronal sulcus.

The wound at the tip of foreskin and the tight foreskin which is there from the beginning are making retraction of the foreskin painful .

In such a situation you will not be able to retract the foreskin even after the wound heals up.
Hence I would advice you get circumcised .

You can take antibiotics like Augmenting and Chymoral forte for 5 more days and use topical antibiotic ointment like Neosporin for application over the wound.

If problem persist you have no option but to get circumcised.

Assuming that if it doesn't heal or heals but still doesn't retract-
I want to go for partial circumcision(just ridged band removal).
I want partial one and doctor has offered me that as well.

--Is there any problem that i can face due to opting for partial circ(and not full circ)?
--Time for recovery and also how many days post-surgery, i will be able to retract it to take care of my frenuloplasty wound?

Thanks for follow up.
You can go for partial circumcision ,You will not face any problem as long as foreskin is freely retracted completely over glans penis so that you will not face any problem in having penetrative vaginal sex in future.

It will take 2 weeks for complete recovery.and wound healing .
