What Causes Pain Below The Rib Cage Radiating To Hand And Upper Back Shoulder?

Question: I am a healthy 90 year old male; I woke up with a pain on my right side just below the rib cage and appears to have radiated up to my right hand trapezius muscle and upper back shoulder. The abdominal pain seems to vary in intensuty but doesn't go away completely.
