Brief Answer:
Fungal in nature

Detailed Answer:
Hello,

A long standing bumps which are itchy and red are mainly fungus. They are fungal dermatophytes. They need an oral antifungal tablets like Terbinafine. Please see a local physician who can give you a prescription. They take a lonter time to cure for 4 to 6 weeks. If you can find any over the counter antifungal cream or powder with Terbinafine or Clotrimazole you can try it as well.

Make sure you wash your shoe regularly. It is dry. The socks have to be cotton which can absorb excess sweat and keep the toe dry.

Hope this answers.
