Brief Answer:
Psychogenic/Anxiety

Detailed Answer:
Hi XXXX,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.

If you would have presented to me as an outpatient with this clinical history,I would have told you that -the internal tremors you have mentioned does not appear to be of any organic origin. These are most likely a manifestation of anxiety.Other way,Tremors are not something internal.By definition, tremors need to be visible movements. So,the nature of the complaint is likely to be of psychogenic origin,in your case.
Mild thalamic stroke 5 years back will not cause internal tremors or upper back pain now.
I suggest you to please start doing regular mental relaxation exercises like meditation/yoga and I am hopeful that your tremors will soon go away.
Please take lessons from a physiotherapist for posture training and exercises regarding back pain.Meanwhile,if back pain is severe,non-steroidal anti inflammatory medicines(NSAID) can be taken for 2-3 days.

If you have any further questions,I shall be glad to have you in follow-up.

How come a number of people iwth Parkinson's say their internal tremors are worse than their external ones?

Why does it happen only upon waking? No other time.

Brief Answer:
This can't be Parkinson's.

Detailed Answer:
Hi XXXX,
Thanks for being in follow-up.

Patients of Parkinson's disease do have very low intensity tremors which are not visible externally, and are referred to as internal tremors. However, even in Parkinson's disease, 70 percent of the patients have a correlation of their internal tremors with anxiety.
You are having no other neurological symptoms,so it can't be Parkinson's disease with just internal tremors.
Further, anxiety tremors can be felt more in the early morning, quite often. In absence of proper sleep at night,a person can have worsening of these tremors,in the early morning.

Hope that I have answered your query. If you have any further questions,I shall be glad to answer else please close the thread, rate it and write a review as your rating will be of help to me.
