Question: My husband had 2 episodes of bleeding from the penis. He went to the doctor but no blood was found in his urine. Psa normal. Digital rectal exam grade 2. The advice was to call again if there was another occurance.
What would cause bleeding from the penis but not in the urine. My husband discovered it when he sat down to take a BM. He felt no pain and did not feel when the blood was passed

there are some things in your description that need clarification, so please explain them to me in as much detail as you can.
First of all, bleeding from the penis means while urinating? Or spontaneous bleeding? From the urethra? I suppose it's the first...
So your husband saw red urine without feeling any pain, right? And he went to the doctor (on the same day?) and did a urinalysis, where no blood was detected. Was the urine still red? Were there any blood clots? What about his medical history? Did he ever have renal stones? Glomerulonephritis? Anything else - even unrelated to the urinary tract?

He was not urinating. spontaneous bleeding from the urethra. My husband is healthy.
No medical problems at this time that he is getting treatment for. His cholesterol is a little elevated but not enough to start medication. We are trying to improve our diet. He does not go to the doctor.
The UA was absolutely clear. The PSA was normal and a digital rectal exam was done.
He showed me a spot of blood about 3 inches in diameter in his underwear. He denies pain in his groin area and denies pain with urination.
He was had spots of blood in his underwear on 2 different occasions in the last 2 months. I am frustrated because everything I read talks about blood from the penis as if it has to be from the urinary system.
so when that turned out not to be the case it seems everyone is stumped. it worries me. I just want some possibilities of what this could possibly be.
he went to the doctor 3 days after the blood in the underwear was found.

I suppose the blood was located close to the penis? If it was close to the anus for example then hemorrhoids could be suspected.

And if the blood came out of the penis and there was no external bleeding site then it was the urinary tract! You've mentioned only a urinalysis, PSA and DRE. Did the doctor do a visual inspection of the urethra (cystourethroscopy)? This is important to identify lesions in the urethra like strictures, diverticula, fistulas, etc. The urinary tract has to be thoroughly investigated particularly the part peripherally to the bladder.

So if the blood was coming out of the penis and visual inspection of the urethra brings out nothing then I'll be stumped!
