What Causes Acne Pores On Face After Using Demelan?

Question: Hello Doctor,
I have Grade-III Acne and i'm currently undergoing treatment for it. I'm on isotroin 20mg and also on chemical peeling . I have undergone 4 out of 6 sittings of peeling and i have seen lot of improvement on my face.
Of late , i have been advised to use demelan and i started using it as per Doctors advise.
I see that small pores of acne getting developed again after using Demelan and again they subside within a day as i use proper sunscreen to my face.
Should i continue to use demelan as its developing small pores again ?

My doctor is advising me to go for Pixel treatement. She is advising me to go for 6 sittings and she says that my face would improve a lot and it would cover up most of the scars that are caused due to the acne.
Could you please let me know whats pixel treatment about / its advantades / side effects / most importantly its cost XXXXXXX ?
Would that be a permenent cure for the scars and does it subside the new scars to form in case new acne appears.

Demelan contains glycolic acid, kojic acid and arbutin, they are lightening agents and will help in lightening dark spots. However as your skin is pimple prone , I would recommend only gels, serums and water based products. Demelan is a cream . Similar ingredients are available in gel formulations (eg: Kojivit gel) which you can try for dark spots instead of Demelan cream. Even your sunscreen should be a gel formulation and not cream or lotion. (eg: Ansolar gel or Photostable gel or Suncote gel). Therefore in my opinion, you may stop Demelan cream. Discuss with your treating doctor.

Pixel lasers are one of the modalities for pitted acne scars. Other treatments include dermaroller, subcision and microdermabrasion. Pixel lasers are expensive and price can range depending on the type of laser and the centre any where from 5000 to 20,000 per sitting. You should ask your doctor how much he charges and what is the machine he uses and compare between clinics. It will help to fill up scars to a certain extend , but your skin will never become baby smooth. Side effects include redness and swelling, pain, sensitivity to sunlight and pigmentation if proper after care instructions are not followed. Advantages: It fills up scars, even fine lines and wrinkles improve, pores also improve. Collagen remodeling continues to happen even for many months after the procedure. The improvement you get will be permanent, but it won't help in decreasing new scars. For that you need to control new acne.
A word of caution- Acne scar revision procedures should not be ideally done when you are on isotretinoin. Complete the course of isotretinoin and after 6 months think of invasive procedures ideally including pixel lasers.
