Hello.
Might be a long standing intestinal infection. Give him antibiotics and painkillers. Take him to your doctor. Some investigations and colonoscopy might be needed.

Detailed Answer:
Hello.
Thank you for posting your query in healthcaremagic.com.
This is Dr. Swarnava Dattagupta answering your query.

I understand your concern.
It seems like he is suffering from intestinal infection with maybe some early features of irritable bowel syndrome.
When patients come with such symptoms, I generally advise them the following-

Soft diet for 3 days. Then normal light diet for another 5 days. Avoid fatty food and junk food. Take lots of fluid orally to stop dehydration.
Tab ofloxacin-ornidazole twice daily for 5 days.
Tab pantoprazole-domperidone once daily before breakfast.
ORS solution if there is profuse diarrhea.
You can consult your doctor for prescription of the above.

Also, after 5 days, take tab Normaxin-RT twice daily for 15 days and lactobacillus supplement for 30 days. You can continue pantoprazole along with. Talk to your doctor for prescription of the above too.

Hope I answered your question properly.
