Regarding your question, I would explain that a silent heart attack, is usually related to a heart attack without pain.

This happens when the nerves which receive the pain from the cardiac tissues are damaged (like in neuropathy especially in diabetic patients, etc.).

Typical symptoms of this type of heart attack is shortness of breath, fatigue, or palpitations. Shortness of breath in diabetic patients is equivalent to myocardial ischemia, and in such case further investigations for coronary artery disease should be done.

Additional symptoms may include palpitations (or a subjective description of “rapid heartbeat”) and fatigue. Patients may describe fatigue as an inability to walk long distances or as feeling a sudden onset of weakness.

Other rare symptoms are sweating and nausea.

Silent heart attacks may also be asymptomatic. They are commonly encountered as incidental findings in routine ECG.
