Question: hi 25 yrs olde male, am suffering from ankylosis spondylits , am taking oncotrex 10 mg weekly once and folic acid for 6 months.
suddenly i got transfered so i changed the doctor, he prescribing daily saaz 500mg , i need to know the side effects of the saaz 500 mg ,
i heard saaz reduces sperm count in male. pls advice me am very affraid of this. shall i ontinue saaz or oncotrex ?

Good evening. Ankylosing spondylitis has very limited treatment options available. How bad are your symptoms:

1. Do you experience early morning stiffness?How about night pains

2. Any other joint pains and swellings have you experienced?

3. Have you ever had any episodes of painful red eye

As for your presciption, You are on very low dose of Oncotrex and Saaz. We normally start oncotrex at 15 mg and the full dose of Saaz is 1 gm twice daily

As per your query, Saaz does reduce sperm count in some males, but that side effect is not seen in everyone. And moreoever it is reversible. So you need not worry about it too much. Both the drugs are quite safe and effective in Ankylosing spondylitis.

It is only after marriage, when you are planning for a child , that time you will have to stop the drugs at least 3 months prior.

But yes, you need treatment for your disease. If it continues to progress, you will face more effects of the disease than the side effects of the drug.

Initially i got knee pain with swellings but intake of 1 months tablet, it got reduced and no pain .

In morning.. i do feel tired but no pain... it would be difficut to bend or walk...

only my neck would be more stiff.

am planning to do swimming ! is it good?? or any other best exercise , could you suggest.

i heard that some diety food help in reducing inflomation?

my doctor is suggesteing me some kind of injection , 200mg around 80K..
only 4 dosages . is it good to take/ would i get relif for some years? please suggest on this.. am confused much.

Good evening. Well you are experiencing some of the symptoms, but they appear mostly mild. The presence of morning stiffness does indicate that the disease is still not under control.

As for your query:

Yes swimming is pretty good for the disease you have. Also you should do aerobic exercise like brisk walking( 3 kms in 30 mts) daily. Spine Stretching and Deep Breathing exercises are helpful too. What you need to understand is, that Exercise in Ankylosing SPonydilitis is as good as a painkiller. It will help to mobilize your body better and you urself will feel more fresh.

As for food, we normally dont ask anyone to restrict the diet. Some patients do get a flare of joint pains once they consume food they are allergic to. As such we dont recommend anyone to go off diet but you have to figure out if your pains increase after consuming any particular kind of food. Kindly Avoid that

As for that injection( Remicade, infliximab)..First thing you need to understand is that it is not the cure. Yes the results are very good and it will benefit you for a while, but we like to keep it reserve for the last option.It has 70-80 % success rate. Whether you need it or not, i personally will have to see the patient before recommending it.

At present, my advise is to continue oncotrex and saaz( in proper dose). Dont worry about side effects, let your doctor monitor that. Do proper exercise.And if you still dnt feel fit enough, then consider that injection. Hope the information was useful.

I was in confused state about the spondylitis.

Ur reply made me clear.. I will surely add up more exercises in my routine

And one more thing.. On taking saaz 500mg daily.. I feel little burden in stomach and in comfortable ..
While I was talking previously oncotrex 10 mg weekly once. I feel better...


So do I need to continue saaz daily. Or shall I ask my doctor about my prob and change obcotrex...

Good evening. Well if you are not able to take Saaz, you should inform your doctor. Stomach upset is the most common side effect of this medicine. It normally settles with time, but if it is not settling you should let him know and maybe switch to oncotrex. 15 mg is the dose we usually start Oncotrex, cause there is no point in giving medicines in half the dose as the disease needs stricter control.
