Question: YYYY@YYYY ...Like a tightness in the testicles and slight dull discomfort along with an irritating sharp pain in the tip of the penis. 41 years old -- no sex in 4 or 5 months -- started about ago. ????????????????? Went to doctor about 7 months ago....would urinate and have to do it again immediately again...same discomfort but not while urinating...no discharge or pus.....check for stds -- good and uti --said white bloods cells were a lil high but ok...no diagnosis was given a Z-pak

You need proper clinical examination and investigations.
Investigations include routine hemogram,RBS,RFT,urine RE/ME,Urine culture and sensitivity,ultrasound of abdomen and scrotum,further investigations can be done if needed.
You was rightly given Z-pack for your symptoms earlier.

Investigations like ultrasound of abdomen should be done to rule out ureteric or urinary bladder calculus.Sharp pain at the tip of penis may be due to ureteric calculus.CT scan of KUB region can also be done as it is the investigation of choice in ureteric calculus.CT Scan is more sensitive.

Beside this USG of scrotum should also be done to rule out any pathology as you have tightness in testicles.
Treatment will be done based on clinical findings and investigation reports.
