Suggest Ways To Manage Urinary Incontinence When Diagnosed With Prostate Cancer

default
Posted on Fri, 23 Dec 2016
Question: I am 64 years old ... Stage 3 prostate cancer successfully treated 7 years ago with surgery / radiation / hormone and mild chemo therapy. My incontinence symptoms have improved steadily to currently dribbling only when standing or walking etc. No leaking when horizontal including when sleeping and overnight. I cannot get to the totally dry state. Is there a definitive test available to determine if the issue is my bladder or something in the urinary tract ? The leakage is always small and I do not have a feeling of a full bladder or an urgency to urinate. Also: While erections are not a problem.....Climax (feeling) can produce urine in amounts that vary depending upon liquid intake. Looking for thoughts ? Next steps... Specialist ? Thanks

Brief Answer:
Incontinence could be due to stricture urethra or Bladder neck stenosis

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that you have undergone surgery for cancer of Prostate followed by radiation and chemo therapy and has now fcing problem of urinary incontinence .

The incontinence of urine following radical prosatectomy in majority of cases is due injury to urethral sphincter during surgery or due to radiotherapy .

You need to confirm this by doing Urodynamic study of bladder and Cystoscopic examination .

Consult your Uro- Oncologist for revaluation and further tests suggested earlier

Ideally speaking there is no curative treatment for this problem .

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

Thank You Dr. XXXXXXX for addressing my query...... Appreciate the quick response.

Looking for clarity on your following words: "Ideally speaking there is no curative treatment for this problem".

What "Problem" ? My next steps would be to follow your direction and conduct the Urodynamic study and Cystoscopic exam. The findings of these procedures would determine if my "problem" could be cured. Your statement leads me to believe that my problem has no cure ("no curative treatment").
Have I misunderstood you ? Please clarify. Thanks again for your time....

Brief Answer:
Incontinence of urine for7 years post surgery suggest irreversible injury

Detailed Answer:
Hello
Thanks for follow up .

The incontinence of urine for 7 years after surgery suggest irreversible injury to urethral sphincter ,
The test I have suggested are to establish and confirm the diagnosis .

As regards my statement about curative treatment I mean there are no medication to resolve the issue for forever .

If the diagnosis is confirmed and incontinence is embarrassing your routine life you will need to undergo surgery to place artificial sphincter so that you can have effective control over urination .

Thanks and Regards.
