Posted on Fri, 8 Aug 2014
Question: Experiencing classic herpes symptoms but very atypical patterns on penis and also on timeframe of symptoms.
Going on day 83 since exposure. Over 6 STD panel tests taken including IGG HSV 1/2 tests. All coming back negative with last test at day 81.
Below is timeline:
3 weeks post exposure- red blemish on penis head
1 month post exposure- flu like symptoms, sore throat, & tired
1 month to current- red penis skin, sore and throbbing, odd raised skin all over penis gland, small white circles semi sub epidermal that come and go. But nothing has blistered, pop, or crusted to date.

Brief Answer:
Kindly provide more information

Detailed Answer:
Hello and welcome to healthcaremagic

I am Dr. Kakkar. I have gone through your query. I remember interacting with you few days ago.

Herpes classically presents with well defined erosions/breaks on the genital mucosa or the surrounding skin. The lesions are painful. The whole clinical picture is fairly characteristic of herpes to be missed or confused, clinically. Herpes is very hard to miss clinically

-I could relate your kind of presentation to some of my patients who practice over enthusiastic hygiene of the delicate genital mucosa causing it to be become red and sore secondary to soaps, antiseptics etc which they may using. I hope that is not the case with you?
-Are the lesions all over the glans? Or are they just on the raised rim? If they are just on the raised rim, they could be just Pearly Penile Papules.

Kindly upload a clear digital photograph of the lesions so that I may be able to guide you better.

Regards

Dr. Kakkar,

Pleasure to speak with you again.

I'm still at a loss for what it is I contracted.

Since we last spoke, I've had another complete round of tests done and still all negative.

I've attached some images in hopes that I could get your opinion please.
The dermatologist, STD specialist, and Infectious Disease specialist all have looked at my penis and said there is nothing they can see indicative of herpes.

Brief Answer:
Use topical steroid+antinfungal cream combination

Detailed Answer:
Hi

I have gone through the photographs.

The things I could see are that you have pearly penile papules and candidal balanitis.

Pearly penile papules present as one or several rows of small, pearly or flesh-colored, smooth, dome-shaped bumps situated circumferentially around the corona of the glans.

They are more common in younger men and those who are uncircumcised. They can be left alone.

Candidal balanitis presents as macerated papules or papulovesicles, which rupture easily to leave behind slightly raw, red areas. There may be itching and some sore sensation.

I would suggest you to use plain lukewarm water for cleansing the delicate genital mucosa. Avoid all soaps and harsh cleansers.

Topically you can use a cream containing a combination of a topical antifungal+steroid cream (e.g clotrimazole + beclomethasone cream combination). You may use it twice daily for a week
